PERSONAL TRAINING AGREEMENT

Congratulations on your decision to participate in a supervised exercise program! With the assistance of Miss Debi, your personal trainer, you will greatly improve your ability to accomplish your training goals faster,

safer, and with maximum benefits.

In order to maximize progress, it will be necessary for you to follow program guidelines during supervised and

(if applicable) unsupervised training days. Remember, exercise and healthy eating are EQUALLY important!

 

During your exercise program, every effort will be made to assure your safety. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. It is highly recommend to get a complete physical and obtain medical clearance from your physician before starting any exercise program.

 

In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise program.

 

Upon entering into an oral/written agreement with Miss Debi for a personal training exercise program, you accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility is assumed by Miss Debi.
 

It is recommended that all program participants work with a personal trainer three (3) times per week. However, due to scheduling conflicts and financial considerations, a combination of supervised and unsupervised workouts

is possible.

TERMS AND CONDITIONS
  • Training programs are purchased in (4) week packages ONLY

  • Social distancing of (6)ft MUST be maintained

  • Masks must be worn throughout the training session (excluding virtual training)

  • All training sessions are (50) minutes in length which includes warm-up and cool down

  • Personal training sessions that are not rescheduled or canceled within 48 hours of the scheduled session time will result in forfeiture of the session at the rate of a single training session

  • Clients arriving/starting late will receive the remaining scheduled session time

  • The expiration policy requires completion of all personal training sessions within 30 days from the date of purchase.  Personal training sessions are void after the time period has expired

  • No personal training refunds will be issued for any reason, including but not limited to relocation, illness, and unused sessions

  • 1x per wk = $60 per week

  • 2x per wk = $110 per week

  • 3x per wk = $150 per week

GROUP (2-3)

  • 1x per wk = $50 per person/wk

  • 2x per wk = $90 per person/wk

  • 3x per wk = $120 per person/wk

GROUP (4-5)

  • 1x per wk = $45 per person/wk

  • 2x per wk = $80 per person/wk

  • 3x per wk = $105 per person/wk

GROUP (6+)

  • 1x per wk = $40 per person/wk

  • 2x per wk = $70 per person/wk

  • 3x per wk = $90 per person/wk
